Here is the course I followed to get a successful install with ReadyHosting, for those that are having issues:

First off you need to pay additional money to get a SQL Database, it is $25/Year.
You can sign-up for a MS SQL 2000 Database via your control panel (Available through the main website www.readyhosting.com).
Choose the 'Add a MS SQL Database DSN' option.
Be absolutely sure you write down all of the information you enter on this page (it is a absolute pain in the neck to get this info later if lost).
Make note of the database name you choose on this screen as it will serve as your DSN during the install.
On the phpBB install screen choose "MS SQL [ODBC]". And be sure to modify the port number to the port specified by Readyhosting when they mail you your database creation confirmation email.
